Does Structured Mean Rigid? Debunking Myths About SCORE Reports

There's a myth that structured formats "fit" the physician and eliminate nuances. However, only two of 34 professionals surveyed expressed reluctance and, after using the system, concluded that the optional free-text fields allow for adding details without losing order [link to the journal article]. SCORE balances the best of both worlds: controlled vocabulary to ensure consistency and open sections for describing specific findings. The result is an increase in interobserver agreement and a reduction in terminology errors—no more confusing "spike" with "point" or "spicule."
